A list in tabular form containing names of chemicals with compositions in bracket, Commodity Codes/Harmonized System (HS) codes, and quantities requested. Material Safety Data Sheets (MSDS) for each new chemical from the overseas manufacturer (only applicable to applicants requesting for new items).
